Office Forms:

Emergency Contact Form – We need this form filled out for every student enrolled in our school. We also need a new form filled out each year.
Excuse Form – Use this form when your child is absent, riding home with someone else or not on the bus, if your child is leaving early for an appointment, etc.
Dental Form – Dental forms are required for students in Kindergarten, 3rd grade, 7th grade, and for new students enrolling for the first time.
Physical Form – Physical forms are required for students in Kindergarten, 6th grade, 11th grade, and for new students enrolling for the first time.
Tylenol Permission form– We are not permitted to dispense Tylenol without a doctor’s signed approval.

Sports Forms:

Sports Health Form – This form is to be filled out by the parent of students playing soccer and/or basketball.
Sports Physical Form – This form is to be filled out by the doctor for students playing soccer and/or basketball.
Permission To Treat Form – This form grants us permission to seek medical treatment when necessary.
